Social prescribing is a key component of Universal Personalised Care and is a way for local agencies to refer people to a link worker.
Our Social Prescribing Service supports you to take control of your own health and wellbeing, make positive lifestyle changes and decide what you would like to do to feel healthier.
The Social Prescriber attached to our surgery is Mel Palachandran. She works with us on Tuesdays and is here to listen to you and connect you with opportunities in the community that might help you feel better. This could include supporting you with things like managing your stress, loneliness, finding social groups, learning new skills, physical activity or getting support with employment, benefits, housing and legal advice.
If you are 18 or over, you can ask a member of staff to refer you to this service for support or read more details about social prescribing.
